Photo Release Form

In submitting my photo, I give Portlandia Foods, Inc. (“Portlandia”) an irrevocable license to use my photo, now or in the future, worldwide, non-exclusive, and in any manner to use, reproduce, distribute, prepare derivative works of, display, and create works that include part or whole of the submitted photo. This includes, the right to modify the photograph as necessary for the bottle labels. This also includes use on magazine advertisements, brochures, websites, and other promotional materials. I also acknowledge that Portlandia is under no obligation to use my photo(s).
By clicking on the checkbox below, I agree that Portlandia will credit my photo(s) on the bottle label with my name, as provided, and my brief title, which may be edited by Portlandia in its sole discretion.
I agree that there are to be no fees, commissions or royalties paid to me for the use of my photo(s).
I acknowledge and agree that I am the owner of the enclosed photo(s) and that I have the right and authority to grant the licenses granted herein.
